official Notice for Aurora Luxe Medical, P.A. including: required header language, uses and disclosures permitted
without authorization, uses requiring authorization, individual rights (access, amend, accounting,
restrictions, confidential communications, paper copy, complaints), the entity's duties, the effective
date, contact information, and the OCR complaint pathway. Provide it to every patient at first visit.

This Notice applies to Protected Health Information held by Aurora Luxe Medical, P.A.. It does not apply to the marketing-only information collected on this website (see our Privacy Policy for that scope).
